Your passport is a valuable document. It should always be kept either in your own custody or in the custody of a person duly authorized by you. If lost, or destroyed, the fact and circumstances should be immediately reported to the nearest Indian Mission/Post and the local police. Your passport must not be altered or mutilated in anyway nor any endorsement made on it by any person other than a duly authorized official of the Government of India. The maximum life of an Indian Passport is ten (10) years from the date of issue. Please ensure that you obtain a fresh passport while the current passport is still valid, as issue of new passport, after expiry of previous passport, could take a number of weeks and may required additional documents and procedures. There is no requirement of minimum or maximum remaining validity of the current passport for application for re-issuance of the passport. Please keep a photocopy of your passport in a safe place. In the event of loss or damage of the original passport, its photocopy helps the applicant in providing important passport details, which are necessary to process the application for duplicate passport. Without these details, issuance of duplicate passport may be delayed. It is an offence under the Passport Act 1967 to give false information in the application. Passport facilities can be denied on grounds of suppression of factual information, submission of false particulars, willful damage of passport and for making unauthorized changes in the passports. Your passport can be impounded or revoked by the Government of India under the provisions of the Passport Act 1967 in which event you will have to surrender the passport to the nearest Indian Passport Issuing Authority. Misrepresentation of facts while applying for the passport may result in denial of passport/consular services. Applications received with incomplete details will remain unprocessed. Every child should have a separate passport. If a holder of an Indian Passport acquires nationality of another country, he/she must immediately surrender his/her Indian Passport forthwith to the nearest Indian Mission/Post abroad. Unauthorized possession of an Indian Passport shall constitute an offence punishable under the Indian Passports Act 1967.
On-line Passport Applications
In accordance with the guidelines of the International Civil Aviation Organization, the Government of India has commenced the issue of Machine Readable Passports. These passports are issued at a central issuing office in India. Applicants are required to register their applications through the online registration form accessible at https://portal6.passportindia.gov.in/Online/index.html and then to submit their registration printout with application and other documents to the Consulate. Since the information is electronically communicated to India, processed there and the final printed passport would be couriered back to Erbil, the entire process could take about 30 days or more depending on backlog in the Consulate and the Central Printing Cell. Applicants are therefore advised to take this into account and apply well in advance for their passports.
How to apply
All applications can be made by clicking on the link on-line registration form at https://portal6.passportindia.gov.in/Online/index.html. After filling in the form fully and accurately, applicants should upload their passport size photographs as per guidelines given. Click 'Save & Print'. You are then asked to confirm the appointment for presenting the form at the Consulate. Please note that this is not relevant for Consulate General of India, Erbil as no prior appointment is required to visit the Consulate. You may simply click "Confirm the Appointment" and proceed further to take a printout of the full form and then to fill in the remaining columns.
The completed forms can then be brought to the Consulate counter on any working day between 0930 hrs. and 1200 hrs. along with the fees and documents/ information listed below: -

All applications for Duplicate passports have to be made in person at the Consulate after on-line registration and completion of application forms. The Consular Officer may ask for any other supporting documentation as may be required after scrutiny of application / interview with the applicant.
Change of Name in the Passport
Same as in Re-issue of Passport cases with following additional documents:
